Out of the strange fog crawls a huge, bloated form, like a balloon of flesh. The skin is lined with visible branching dark tubes (are they veins or nerves?), the body swells and shakes like an inflating beach ball. The creature waves two cord-like tendrils, lashing one out and snatching the party wizard. As the fighter rushes to save him a muscular, serrated tail reached down and takes his head off. Just as your casting holy smite, you feel a sharp pain in your back and turn around to see the rogue, who vanished earlier, stabbing you in your gut. The last thing you see as you fade out is the creature staring down at you with its one, menacing, proportionally small, glaring eye.

The title "Crawling Eye" is a misnomer, as while the creature crawls it is most definately a large body with a distinct and seperate eye. Perhaps it was a mistaken identity since it is rarely seen outside of a concealing mist and the person had the good sense to flee before getting a clear look at it.

Crawling Eyes have a reptutation for being beastial, evil creatures that prey upon local life. While there are more evil ones than good, the selfish and muderous number has been greatly exaggerated, and usually is the only contact since most good and neutral Crawling Eyes prefer to stay in the lonely mountain tops and shun humanoid contact.

The home of the Crawling Eye is unknown. Most believe it to be from some unknown plane or the far realms, although some scholars say they hail from a distant land across the void that borders the edge of our planet, or simply from the icy alpine regions of this world.

Combat: Most crawling eyes encountered in combat are seeking to head down from their mountain and invade the neighboring communities. They tend to roam about and prey on climbers and adventurers, staying near and infecting them with their corrupting mist. When in melee they try to grab the most dangerous opponents with their tentacles and use tranformed minions to kill the rest.

Constrict(ex): A grappled creature takes 2d6+7 constrict damage each round.

Radioactive Mist(su): A crawling eye generates its own atmosphere so it can breath, which extends for roughly a mile radius. In addition to allowing the eye to breath, these mists have a terrible effect on animal life.
Humanoids, giants, beasts, animals, and magical beasts must make a fortitude save (dc:20) each hour spent within the mists or be transformed into eyespawn.

Vorpal Tentacle Rake(Su): A crawling eye's tentacle rake is treated as a vorpal weapon.

Heat Vulnerability(ex): A crawling eye suffers the effects of a hot climate in temperatures of 46 degrees or more. Areas of 82 degrees or more are extreme hot climates. They take double damage from fire.

Alien Respiration(ex): A crawling eye needs very thin air to breath. When at an elevation less than 500 feet above sea level it must make a fortitude save every hour or be slowed until it can get at least four hours of rest in an area more than 500ft above sea level.

Psychic Broadcasting(pi): Any psion that sees the crawling eye's cloud from 10 miles away or less will feel a strong compulsion to go to its general location. Whenever they manifest a clarsentience power with a target, they must make a psicraft check dc:20 or manifest the power on the crawling eye.

Interplanetary Flight(ex): A crawling eye can traverse vast airless voids (such as the negative and positive energy planes and the quasielemental plane of vaccum) safely in its own mist cloud without fear of suffocation.



Eyespawn Template
This template can be applied to any humanoid, beast, magical beast, animal, or giant. The creature's type changes to undead, and it gains the cold subtype.
Hit Dice: Change to d12
AC: As a creature becomes an eyespawn its flesh crystallizes, increasing natural armor by +1
Attacks: A single slashing weapon possessed by the creature becomes a vorpal weapon when weilded by that eyespawn. This is supernatural.
Special Attacks: The creature can use detect psionics at will as a psi-like ability.
Special Qualities:
Virtually Mindless(me): While the creature can maintain a semblance of intelligence and original personality, it is truly nothing more than a soulless crude vessel of the crawling eye's will. As such, it is immune to all mind-affecting effects other than those of crawling eyes.
Psychic Receptance(me): Any crawling eye within 300ft of an eyespawn can send that eyespawn a telepathic command. That command is treated as a suggestion spell, except the eyespawn has no will save to resist. If there are two conflicting commands given, the crawling eyes must make opposed charisma checks to see which is carried out.
Undead Traits
Souless: A creature's soul leaves its body and departs for the afterlife when it becomes an Eyespawn. They have no soul and as such are not valid targets for a magic jar spell. The soul is free for ressurection, but magic requiring a body or remains such as raise dead and ressurection will not function, since the body is currently animated by a forgeign agent.
Abilities:Str+4,Dex-6,Con-,Int-2,wis-2,Charisma is reduced to 1. With their flesh crystallized and nerves frozen and numbed Eyespawn have virtually no manual dexterity or fine motor skills, and even something as simple as pouring a drink or lighting a pipe is a vexing challenge. Eyespawn are mentally dulled by their transformation, and in they change they have spiritually died leaving behind no shard of will, spiritual strength, and with only a crude simulation of personality. They are however given increased physical strength by the change due to an increase in the amount of body mass that can move and a change in the fundemental nature of their physiology.

seems u forgot to add the feat-bonuses: init is +2 total, will save +10 total. the attack bonus of vorpal tentacle rake is missing, should be +5, cause it has no multiattack feat.
maybe a higher cr is better, it creates spawns that fast, and even players with level 7 would get spawns fast. 3 failed saves required to morph, is maybe more player-friendly...
Maybe this could be a guide:
Radioactive Mist: Every 15 minutes a creature in the radioactive mists must make a Fortitude save against DC (10 + 1/2 HD + Con.mod, i guess) 20. If it fails this save 3 times (shortest period 45 minutes) it becomes an eyespawn.
1st failed save: Lose 2 Dex, 2 Con
2nd failed save: Lose 4 Dex, 6 Con and -1 penalty to will saves, +1 natural amor
if any creature dies through these effects, it becomes an eyespawn instead. as long as the creature isnt fully transmorphed, its former state may be regained via the remove disease spell.

In the sense of being big, round, betentacled, and having some manner of seeing, yes, but the resemblance ends there. Only one eye, no mouth, no chitinous plates, etc.
