Decapiti
Decapiti are headless corpses animated by magic, similar in nature to skeletons and zombies. Uncontrolled decapiti usually seek some dark place to hide and lurk about, moving after creatures with interest and avoiding light sources. On their own they only have a 50% chance of retaliating against an attack. Decapiti, when commanded by a necromancer, have a cumulative 1% chance each night left without instruction of forgetting any commands beyond a basic single-condition two-art statement (such as "attack any life here other than me" or "load barrels into the wagon until the wagon is full").

Because of their limited ability to percieve their environs and react to approaching threats, decapti are usually used in the company of other undead.
Decapiti Template
This template can be added to any living creature other than an outsider or elemental with a corporeal body and a head. The creature's type changes to undead.
HD: change to d12. This template cannot create undead with more than 13 hit dice, those with more have their hit dice reduced to 13.
Speed: reduce land speed by 10ft (minimum 5ft)
AC: gains +3 natural armor (if none) or +2 boost to existing natural armor
Attacks: the creature gains two slam attacks dealing damage appropriate to its size. It retains natural attacks other than bite or gore attacks.
Special Attacks: The creature loses all special attacks other than improved grab and trample and gains a frightful presence attack.
Special Qualities: the creature loses all traits other than physical damage reduction, energy or elemental subtypes, and energy restances of 15 or greater. It gains daylight vulnerability, DR5/slashing, blindsight 30ft (naturally blind and deaf), cold resistance 10, and undead traits.
Abilities: Str+4,dex+4,Con-,Int-,Wis8,Cha2
Feats: Improved initiative and toughness (bonuse feats) (in case the facts that every single mindless creature loses all feats other than bonus feats, and there are no other feats in the sample creature, and that there is no text saying "retains base creature's feats" weren't enough of a clue, you should know that the creature loses all of its original feats and gains only these two bonus feats in return, and that from your apparent intellect you are a blight on society and deserve to be injected with mental stimulants and bludgeoned with "The book of exalted deeds", "the book of vile darkness", and "draconomicon.")

Casting this spell causes a target headless corpse to rise as an undead decapiti under your control. The creature must have been killed by decapitation, so the caster cannot simply cut the head off of a corpse.
Material component: a carved black onyx skull worth 500gp and a silver neck brace worth 137gp.

Ah, a headless undead! How refreshingly classic!

Abilities: Str+4,dex+4,Con-,Int-,Wis8,Cha2
Feats: Improved initiative and toughness (bonuse feats)

You should note that, being mindless, it also loses all feats except the two bonus feats.

What, exactly, is the base creature? I suspect it's a 1st-level human, but I can't tell for sure. What are or were its abilities?

:P I think what you meant was that the creature must have been killed by decapitation. Also, why the odd GP value for the silver neck brace? Why a neck brace at all? Without a head...

The neck brace is the material components made more interesting than the same bloody black onyx gems. I made it slightly less than the max GP that would be required for a 13HD corpse, even though everybody knows that all undead hit dice are worth the same, and it's perfectly reasonable to require the same 50gp PER hit die for a mindless, basic, easily-turned undead as for a sentient spawning turn-resisting super-powerful level-sucker.

Other changes recommended have been made in a highly bitter, sulky, and grudging manner.

I think it was one of my hard, achey, overly touchy nights. I've been having a bad month and a lot of terrible days and worse nights in between. =/

Sorry for lashing out.
