A monstrous insect the size of a horse crawls nervously acrossed the ceiling overhead. Its large, compound eyes stare unblinkingly as the vermin cocks its head to the side and vibrates its transparent wings. A disgusting fluid drips from the creature's bladelike mouthparts just before it releases its grip on the ceiling and dives toward you.

A dire fly is the adult form of the larval dire maggot. Although generally solitary, dire flies occasionally gather in large swarms when food is plentiful. Like house flies, they feed primarily on carrion and garbage but their serrated mandibles and keen vision make them capable hunters. An adult dire fly is about 9 feet long.

Combat:
Dire flies typically scavenge for food but are easily excited by the scent of blood. Once a dire fly has spotted potential prey it rushes to the attack, biting viciously and draining the foe of its life's blood. Opponents smaller than the dire fly are seized and carried aloft so the insect can feed on the wing.

Blood Drain (Ex): A dire fly can drain blood from a living victim with its mouthparts by making a successful grapple check. If it pins the foe, it drains blood, dealing 1d4 points of Constitution drain and automatically deals bite damage each round the pin is maintained.

Disease (Ex): Red Ache - bite, Fortitude DC 15, incubation period 1d3 days; damage 1d6 Str. The save DC is Constitution-based.

Improved Grab (Ex): To use the ability, the dire fly must hit an opponent of Medium size or smaller with its bite attack.

Wide Vision (Ex): A dire fly's compound eyes grant it a +4 bonus to Spot checks (already calculated into the stat block) and prevents it from being flanked.

*Skills: +8 racial bonus to Climb checks, +4 bonus to Survival checks when tracking by scent.

I like it! Only things I would change about it is make it have kind of an acidic bite with maybe a short range acid spit attack instead of the blood drain.
When all do we get some dire maggot stats?:P

I'm glad you like it. I gave it blood drain because I was basing it off of the horse fly. I made a giant fly a few years ago that had an acid attack and I even considered giving it to my dire fly, but I figured that it was powerful enough with all of its other special attacks. Maybe I'll make a re-vamped version and I give it to it later. I dunno. :D
