I noticed that some monsters with damage reduction have a "x" AND "y" listed for ignoring the DR, while others have "x" OR "y". Does the first one mentioned mean fx a balor's DR only is ignored if the weapon both cold iron AND good? Or is it just an error which should have been an "or" instead of "and"?

The "and" is correct. See p.307, 308 of the MM 3.5, or search the SRD for a proper explanation of how 3.5 DR works.

Cold Iron or Magic.... My cold iron longsword is just as effective as your +1 longsword at overcoming.

Cold Iron AND Magic... Neither of us bypass it, but if one weapon was a +1 Cold Iron Longsword, it would work very nicely. This way you'll notice that certain monsters that are "larger" versions of other monsters will have ORs that turn into ANDs as the CRs increase.
