Since my Dust Zombies proved to be fairly popular and there was a decent demand to see them as a template, I've decided to do so.
The Original Dust Zombies can be found in my signature if you are interested in seeing where this template evolved from.

Dusty Dead

Dusty Dead are the ancient animated remains of those slain by Dust Zombies, or undead who have survived so long their bodies begin to crumble to dust.

Creating a Dusty Dead

Type: Changes to undead, or stays undead.

Hit Dice: Changes to d12, if already undead no change.

AC: Dusty Dead gain a +6 to natural Ac, if they already have a Natural Ac use the better value.

Special Abilities:

Dust Cloud: Expanding in a 5 foot radius around the dusty dead is a choking cloud of dust and ash, anyone entering this cloud must make a fortitude save DC 10 + 1/2 HD or takes 2d4 damage and is nauseated for 1d4 rounds as they cough out the dust. A successful save only grants protection for 1 round and a new save must be made every time the cloud is entered and at the start of the dusty deads round. If someone attempts to protect themselves, such as covering their nose and mouth with a wet rag, they gain a +4 circumstance to their fortitude save.

Breath Weapon: As a full round action the dusty dead can exhale a cone of ashes, this is a 25 foot cone that deals 6d4 damage. A reflex save DC 10 +1/2 HD for half damage can be made, a failed save requires a fortitude save DC 10 + 1/2 HD as has the same effect as entering the undeads dust cloud. It may use it's breath weapon once every 1d4 rounds.

Dust Form: Whenever the dusty dead is damaged it explodes into a cloud of dust, while in this dust form the dusty dead gain a fly speed of 40. The dusty dead will simply move in as many squares of it's opponents as it can taking a double move. It is still surrounded by it's dust cloud in this form. While in it's dust form the dusty dead is immune to weapon damage. It reforms in 1d4 rounds after exploding. While in it'd dust form the dusty dead can not use it's breath weapon.

Death Throes: Once slain the dusty dead burst into a cloud of dust and ash in a 30 foot radius. Everyone within the area of effect must make a fortitude save DC 10 + 1/2 HD or takes 2d4 damage and is nauseated for 1d4 rounds.

Special Qualities: Gains the undead subtype.

Abilities: Dusty Dead Gain a +4 to Str and Dex, they lose their con score (if any) and their Int score (if any) they have a minimum of 10 wis and their cha is reduced to 3.

Skills and Feats: As the dusty dead has no Int score it has no skills or feats based on it's hit dice, it gains Toughness and Improved Initiative as bonus feats.

CR: As base Creature +4

Why do you have a flat DC 15 for all of its abilities? Usually the DCs for creatures of varying abilities and HD goes 10+1/2 HD+key ability mod. That aside, I like the concept.

Thank you, the flat dc was because the key modifier for breath weapons is usually con, so I opted to make it a flat DC instead of a scaling DC since it would take a serious hit from a score of 0.

Though I might consider making it 10 +1/2 HD with no key modifier, what do you think?

Sounds reasonable. I believe that absent abilities (non-abilities IIRC) do not impose a penalty. They are treated as a +0 modifier, otherwise undead and constructs would have crappy hitpoints. ;)

Also, undead use their charisma in place of constitution as the modifier for concentration checks. I am not sure of this, but it might follow trend to make breath weapons charisma based. (Breathing for undead becomes a matter of your will to breathe rather than your lung capacity, yes?)

Very cool. I am running an (almost) all undead campaign, and I only have MM1, so I was desperately needing some new ideas for undead. THANK YOU! Your ideas have been a life saver!

No problem, there is still about 8 critters I haven't posted yet so you might want to keep an eye out for those.

Nonabilities are like a +0 bonus
If the relevant modifier for a given ability (such as the breath weapon) is a nonability, you use your Charisma modifier or +0, whichever is higher. Check out the vast majority of undead with special attacks that you would normally attribute to Constitution and see what I mean - it's all Charisma-based!
