I am afraid I might have been reading most MM entries wrong, with regards to attack bonusses and more than 1 weapon.

MM states that normally, a second weapon recieves a -5 penalty (mosters with multiattack? only get a -2 penalty).

The Ettins BAB is +12/+7 , but it also states that they have superior twoweapon fighting (2 heads) and do not get any penalties at all to attack and damage rolls with weapon nr2.

So , assuming the Ettin makes a full attack, how many times and with what BAB does he attack? I am thinking +12/+7 for his left arm, and +12 for his other arm, but this seems wrong somehow.

+12/+7 and another +12/+7 seems way overpowered, and I cannot imagine it is capped at the listed +12/+7.

Also, are the feats that monsters have already reflected in their stats, or do we have to look at those separately?

Ettin are correctly written, it's +12/+7 with each arm with their superior 2 weapon fighting and 2 heads (special ability) they also have the full strength for each if I remember :D
If you give them some levels the number of attack goes up for each arm.
There is one in the module "Bastion of broken Soul", something like:
Half red dragon ettin barbarian 10 with two battle axes, eight attacks a round....ouch
Some feats are (great fortitude, feats that gives skill bonus, focuses...) some are not (combat expertise, power attack).
