The Invasion is coming. Worldbreakers move slowly across the land. Spirit Takers steal people away to the Far Realms. We can fight back against them, but the warriors of the Far Realms ate dire opponents. They come on land. They come on the seas. They rain from the skies. The Skyreavers are the first of the invaders that any realm sees. They come and they rain death and destruction. Many Skyreavers have been seen to gang up on and bring down even the strongest dragons. Skyreavers seem to be cylinders, one end pointed and split for a head and the other elongated into a whip-like tail, with leathery wings. And razor bladed claws.

Combat: Skyreavers swoop and dive, gleefully attacking with their claws, though they prefer to use their tails.

Reality Slash (Su): Any creature struck by the Skyreaver’s tail must make a fortitude save (DC 21) or die as it’s soul is severed from its body and flung into the oblivion of the Far Realms. The body disintegrates the next round, evaporating first into steaming Far Realms ichor and then nothing. A creature so slain is beyond the reach of any magic less than Wish, and it takes three wishes to restore the character: 1 to recreate the body, 1 to restore the soul, and 1 to bring them back together. In each step there is a 50% chance that the wish fails, putting the character beyond any means of returning save perhaps direct deific intervention (and even the gods may be powerless in this matter)

I like the monster but the CR is a little too low for the realality slash. If this is a created monster then you need to improve the CR by about 5 or so otherwise if this is not a created monster its still way too powerful for its CR.
