Does anyone have any outstanding opinions about this Fiendish Codex 2 prestige class? I just read over it thuroughly for the first time and I was wondering if anyone thought it was particularly good/bad/balanced/cheesy.

I like it, at least on paper (I haven't had the opportunity to play one). It seems pretty similar to the core concept of a paladin, but with a much better-executed mechanic. I'm a big fan of per-encounter resources, and I think this class gets enough and ones that're good enough to be worthwhile. The power levels also seems pretty appropriate.

Something to watch for is Binder 1/xxxx/Hellreaver characters, since one of the vestiges lets your attributes heal at one point per round.

My, that is a handy build. It's the same kind of thing to do with a Hellfire warlock. Things just got REALLY interesting in my campaign...
