Using Entangling Ectoplasam and manifesting Grease would bring the enemy's speed to 1/4. Then manifesting Heavy Earth would cause them to move at 1/8 their speed and only able to take one move action. So a creature with 40 speed would have a full movement of 5 feet, which they couldn't even take for the one round when they stand up. It might seem a little excessive, Time Hope essentially does the dame thing with less PP, but I think it is more satisfying.

However, if I do this against my PC's, what modifiers should I take into account if they are Entangled and slowed, trying to get up from prone in grease at 1/8 their movement? The halfling at 20 ft would have an effective speed of 0 ft. rounded down--are they even able to take a move action to stand up from prone in such a state?

Modifiers? OK, just be careful with grease. If a PC rolls around in grease for a round first, then he should get a bonus to his AC versus entangling ectoplasm. The slippery grease would partially counteract the sticky ectoplasm.

Similarly with heavy earth. Greased heavy earth will still be slippery, possibly allowing a PC to slide on his belly faster than he could walk at this point. Of course, grease has a much smaller area than heavy earth, so he won't get far.

Just make sure you use entangling ectoplasm first.

Also keep in mind that attended objects can be greased, disarming or possibly embarrassing the PCs.

Yes. Standing up from prone isn't walking. They can still do it.

Minimum Movement

Despite penalties to movement, you can take a full-round action to move 5 feet (1 square) in any direction, even diagonally. (This rule doesn’t allow you to move through impassable terrain or to move when all movement is prohibited.) Such movement provokes attacks of opportunity as normal (despite the distance covered, this move isn’t a 5-foot step).
I think this answers your question.

But since they can't take full round actions (due to heavy earth), if that hypothetical halfling wants to move 5 feet then he has to start the movement as a standard action and end it as a standard action on his next turn.

He could still rise from prone, though, since that doesn't entail actually going anywhere. Unless one of the effects involves changes "rise from prone" into a full-round action.
