It states that the Goliath is a monstrous humanoid with an LA+1 which is unable to use armour (not proficient). Does this mean a character starts with an effective class level of monstrous humanoid where HD are concernd?

EG: a lvl 1(2 inc LA) goliath fighter would have D8+D10 HD?

Level Adjustment is not a hit die. Level adjustments are, to put it simply, fake levels. They add to the ECL of a character, but provide no benefit. A first level goliath fighter (ECL 2) would have 1 10-sided hit die.

No, it just means that the goliath is of the monstrous humanoid type, but it does not actually have any racial HD. This also means that it gets all benefits of being a monstrous humanoid, such as darkvision, but proficiencies still accrue from its class lvs.

Also note that you can still use armour even if you are not proficient. Masterwork studded leather has no ACP, for example, and thus, you get no penalties even when you are not proficient.:)

I'd also like to add that in the case of applying templates to creatures that have no actual racial HD, they are still treated as having 1 HD (in case you were trying to apply, say, the Feral template, then they'd get all the benefits from the template that would normally go to a 1 HD creature).

Just thought I'd throw that in there.
