Hideous Creature

A Hideous creature, no matter its form, is always horrid to look at, and the twisted personally that so often follows them is very obvious. As such, most see these creatures as monsters, nothing but to be killed, and are often destroyed at birth. Generally they are the result of arcane meddling with the mother to adjust the baby into a more effective killer.
Though some aberrant qualities are easier to hide than others, and horrid creature must succeed on a DC 10 disguise check to avoid revealing it’s nature as per the twisting of the creature’s personality if the person interacting with the horrid creature is aware that horrid creatures exist.

Creating a Hideous Creature
Hideous Creature is an inherited template that can be added to any humanoid. The hideous creature template can be applied more than once to any given creature.
Size and type: A hideous creature’s type changes to aberration. Do not recalculate base hit dice, saves, and skills
Special abilities: Hideous form: a hideous creature comes into existence with one of the following form changes, chosen randomly at first level: tentacle (01-10), whip tail (11-20), club tail (21-30), spare mouth (31-40), knuckle spikes (41-50), sword arm (51-60), spare arm (61-70), thick skin (71-80), sharp teeth (81-90), or horns (91-100).
Tentacle- four tentacles protrude from the creature’s back, seeming to flail of their own volition. If the creature forgoes all other attacks, it can deal 1d3 + 1 damage on each hit, with up to four attacks. Also, it gives the horrid creature a +2 bonus on grapples and climb checks
Whip Tail- the base creature gains an attack equivalent to that of a whip (range 15 ft., 1d3 non-lethal damage, slashing damage) that can be made as a secondary natural attack. if the attack is taken alongside weapon attacks, the whiptail attack is at a –2 penalty
Club tail- the base creature gains an attack equivalent to that of a club (1d6 damage, bludgeoning damage) that can be made as a secondary natural attack. if the attack is taken alongside weapon attacks, the club tail attack is at a –2 penalty
Spare mouth- The creature’s stomach is taken up completely by a large mouth, with sharp teeth capable of tearing flesh from bone. A bite attack can only be made in light or no armor and deals 1d8 damage on a successful attack.
Knuckle spikes- a hideous creature with the knuckle spikes horrid form deals unarmed damage as if a creature of one size larger and deals piercing damage rather than bludgeoning damage
Sword arm- at all times a sword armed horrid creature is considered to have a natural attack that is equivalent in all ways to a longsword (1d8 damage, 19-20 crit., slash damage). The creature takes a –2 on all skill checks requiring the use of hands. All other hands are considered off hands for the purpose of battle.
Spare arm- a hideous creature with a spare arm has a third arm placed under another arm, and it is considered an off hand.
Thick skin- the hideous creature has thicker skin than normal for it’s race, and is considered to have +2 natural armor.
Sharp teeth- The hiseous creature gains a bite attack that deals 1d6 damage on a successful hit.
Horns- the hideous creature gains a gore attack that deals 1d6 damage on a successful hit, or 2d6 damage after a charge
If the hideous form of a hideous creature gives the hideous creature an attack equivalent to a weapon, any feat taken by the hideous creature that benefits the use of the specified weapon, it benefits the natural weapon as well (if a hideous creature takes weapon focus (longsword), then it gains the same benefit for it’s sword arm if it has such).
Alignment: Usually non-good
Abilities: STR +2, CON +2, WIS –2, CHA -2. though hardy and strong, a hideous creature’s mind is twisted and mutated.
Level adjustment/ CR adjustment: +1/ +1

Hideous creature characters
Most hideous creature characters take levels in barbarian or rogue, which serves well with the outcast character type. Sometimes they are trained as fighters, but they are rarely spellcasters due to their penalties on mental ability scores. Paladins are extremely rare because of the need for training that the creature is unlikely to get, and the need for a good alignment.

Gotta use a different name. The Horrid template is found in the Eberron Campaign Sourcebook, and is also used to make horrid creatures.
