When you create a character with a racial level adjustment, +2 for example, you start the character with one class level to give it an ecl of 3. What hit die do you roll for the +2 level adjustment? Is it just the hit die for the starting class?

You don't. Level adjustment doesn't give you hit points. It doesn't give you skill points, base attack bonus, or saving throw bonuses either.

And that is why LA is problematic.

It also doesn't give you feat or ability score progression, so a ECL 3 character with 2 LA has the feats of a first level character, and when he levels he doesn't get an ability score increase.

How about trying to figure out the racial HD progression, where the particular monster as a race gains those special qualities written in MM as they advance?

That way you don't have to worry about LA, since your race doesn't get his most powerful abilities from the get-go but through advancing to higher HD.

Though this might be a problem since most people would want to play a monster race with all of its possible powers rather than to gain them through advancement.

He will actually be of 3rd Character Level so he will gain all the benefits that are gained according to CL.
This means he will already have gained 2 feats and at the next level he will get the ability point.
He does not gain any class abilities.

The idea of gaining the bonuses while advancing in levels and to start him as a LA 0 character is good. But again he has to pay a price for all the bonuses he will gain.

While the character has an ECL of 3, it has a CL of 1. So it will be 2 levels before he gets his next feat and 3 levels before the ability point.
