What happens if one Dvati half gets infected by a lycanthrope while his or her other half remains uninfected?

Unfortunately, this isn't possible. If your lycanthropy comes from an infection, you aren't contagious. I suppose that the infected one could have a child (after being infected, naturally), let the child manifest lycanthropy, then get the child to bite the other twin, but otherwise, there's not much that you can do.

considering they share hitpoints to a certain degree, and when you heal one, you can share the healing between them... I'd assume the infection would go across too

Gah, forgot inflicted lycanthropes aren't contagious. Yeah, I guess they'd find a way to get cured as soon as possible.

Why? Through what reasoning and through what mechanism? Do they share poison? non-magical diseases? curses?

i would assume for ease. they both get it. otherwise the player must take and copy the char sheet and then apply each the two bodies, which makes diseases and poisons and stuff wonky.
