I'm looking at Cloudy Conjuration, from Complete Mage, and I want to make sure I've got it right.

5ft radius cloud - that's 4 squares, right?

The cloud can be placed adjacent to the target - can it be placed so that none of the four squares overlap the target?

Does a creature in the cloud both have concealment against attacks coming in, and have to deal with concealment against attacks going out?

Does the target take the Sickened penalty against the spell being cast?

5ft radius cloud - that's 4 squares, right?Right.

The cloud can be placed adjacent to the target - can it be placed so that none of the four squares overlap the target? It seems to me that it could. The text reads "placed" not "centered" or anything, so, sure.

Does a creature in the cloud both have concealment against attacks coming in, and have to deal with concealment against attacks going out?Yes.

Does the target take the Sickened penalty against the spell being cast? The text reads: The cloud appears in conjunction with the spell taking effect not before or after.

It is up to the DM. I would say that no, it doesn't apply. You'd have to already be sickened to have a penalty. (First sickened, then make the save.) But you can't have that first, since the two effects happen at the same time.

It is powerful enough without giving that too.
