SRD states that I cannot hide unless I have cover or concealment. What if I'm above him (read on the ceiling) or under him?

Draw a line from his space to yours. Is there anything along that line that provides cover or concealment for you?

If not, then you can't Hide.

Of course, "hiding on the ceiling" might be a special DM-adjudicated case, depending on how likely the opponent is going to look up... perhaps in this case it'd be a Move Silently check.

Otherwise, it'd just be a start of combat, with a surprise round as usual, only you're on the ceiling. They'd be flat-footed (you surprised them), but aware of you (they hear you leap down?).

There are rules in the CAdv for remaining hidden even while running out from cover. It is supposed to represent trying to stay out of field of vision just long enough to rush up or to a new position. It lends a little flexibility to the DnD premise that you are always looking equally well in all directions at all times as long as there is no cover/concealment/environmental cap on distances. It is the first time that fallacy/oversimplification has been broken down. What you want is even further...so much so that it essentially becomes DM fiat.

What it really comes down to is assigning penalties to your opponent's DC0 check to see you in plain sight, so far that he can fail. Being high, being in an unusual place, etc. help, but only a little. Hanging from a 20ft ceiling would probably be DC7 (2 for distance, 5 "distraction" for being in an unexpected position).
