Blooded [Regional] +2 Initiative and Spot checks, cannot be shaken (Player's Guide to Faerūn)
Thug [Regional] +2 Initiative, Appraise, and Intimidate (Player's Guide to Faerūn)

Note that as regional feats you can only take one, and you must be able to select the starting region for your campaign or have the DM's permission.

Danger Sense (from Complete Adventurer) doesn't increase your initiative, but it does let you reroll an initiative check once a day.

4 Improved Initiative feat (PHB)
4 Roaring Armor enhancement (MIC)
2 Eager Weapon enhancement (MIC)
5 Warning Weapon enhancement (MIC)
2 Aggressive trait (UA)
2 Blooded Feat or something like it (various, PGtF)
2 Armor of the Watching Master (MID, 2 pieces)
2 Battle Hardened feat (RoS)
2 Shifter Instincts feat (RoE)
2 Quick Reconoiter feat (Cadv)

Initiative is a Dex-check so anything that increases Dex and Dex checks also helps.

Best I got was 22 with Primal Instinct (spell, dragon magic), improved init., and 13 dex, but that wasn't trying.
