I don't have my Monster Manual handy, but if I remember correctly, there are two different level adjustment options presented, one for with and one for without.

The additional level adjustment you're thinking of (+6 instead of +4) is for Pixies who can use Otto's Irresistible Dance once per day as a spell-like ability.

Yes, but probably not the way you're thinking. As a pixie, you gain the ability to craft these magical arrows, without taking any crafting feats. You still have to pay for raw materials and spend XP to create sleep or memory loss arrows. I don't know how much they cost but I am pretty sure they are priced in Savage Species (and possibly some other sources as well).

Ahh, thanks. I knew there was some variable ECL option, but pixies aren't OGL >.<

They are! (http://www.d20srd.org/srd/monsters/sprite.htm#pixie) They're just hidden under "Sprite" rather than getting an entry of their own. ()

Many of the monsters in the SRD/PHB get grouped thusly. Why the confusion?

SS lets them craft the arrows, IIRC.:)

With the 3.5 revision when a pixie is used as a character it loses the special arrows ability. The MM errata also removes polymorph from their spell-like abilities.
