From the SRD:

When using the Deflect Arrows feat you may catch the weapon instead of just deflecting it. Thrown weapons can immediately be thrown back at the original attacker (even though it isn’t your turn) or kept for later use.

You must have at least one hand free (holding nothing) to use this feat.

Does this give you an extra attack out of turn on top of your usual attacks? If so, is the feat balanced?

Yes (that's exactly what it says, isn't it?), and yes. Chances are, you won't be facing someone that throws things. And when you do, you probably won't have all of the pluses that they do, or that you do with your primary weapon. There's not even a guarantee that you are proficient with it. It's 1/round at most, and besides, it requires a good amount of feat sinkage.

yup, and from the loooooooooooooooong term view, it isnt even a prerequisite for the Exceptional or Infinite Deflection Epic Feats.
