You can't be tripped if you're already on the ground.
You need to wait until the person tries to stand, at which time you can attempt a trip with the attack of opportunity you get for them attempting to stand.
For example - with Improved Trip, you can trip someone and then attack them, but you couldn't successfully trip someone and then use your attack to try to trip them again. If they stand up, then you are allowed to try another trip attempt.

The only way to keep someone on the ground is to ready an action to trip them when they try to stand - But then if they roll out of your reach before getting up or if they simply remain on the ground, then you won't get the chance to try to trip them.

Similarly, there are some things that cannot be tripped. You cannot trip snakes, gelatinous cubes, or similar things (I believe it was covered in a sage advice).

Correct.

This is incorrect. AoO are resolved prior to the action that provoked them, thus, AoOing a tripped foe standing up is resolved before the foe stands up. and since you can't trip a prone foe, and the foes already prone (or he wouldn't be standing. Also, he maintains the prone penalties for this AoO), you can't trip the foe when he stands. I believe this is stated in the FAQ.

this is correct. though them rolling out would provoke an AoO, and they can only move 5ft while prone, meaning they may not be able to get out of your reach (if using a spiked chain and/or enlarged).

I think the ruling is, anything that relies on limbs for it's current form of locamotion can be tripped

You can not use a trip attack when the person is trying to stand up this has been answered in the following FAQ question:

When a character gets up from prone, when does the attack of opportunity take place? When he is still prone? When he is standing? Can the attacker choose when to attack? In one case, the attacker can get a +4 bonus to hit. In the other, he can make another trip attack.
All attacks of opportunity happen before the actions that trigger them (see Chapter 8 in the Player’s Handbook). When you make an attack of opportunity against someone who’s getting up, your target is effectively prone, and therefore cannot be tripped. You could ready an action to trip a prone foe after he gets up, however.

A snake can't be tripped.
A prone biped can't be tripped.
