Since permanent insanity is the equivalent of death, you might want to make the insanity spell 9th level, as permanent insanity can only be removed with a wish or miracle spell.

hmmm....i think i remember reading in a DUNGEON mag, number....huh, can't find it...well, i'll tell ya this...it was in that Adventure Path thing, like the third step, maybe? and this paladin, name's Alex Tercival or sumthin...ah dangit, lemme just find it...hey hey, not like it was sitting out or nuthin...ok, DUNGEON 104, dang i'm good, Alek Tercival is the name of the paladin that went insane...his effects, though were considerably different than that of the spell...not merely a confused state. here it is..." a -2 penalty on attack rolls, skill checks, and saving throws...also confers a -6 penalty to his wisdom...actually curing his insanity requires a heal or greater restoration spell". now, idk if that's at all what u were lookin for, but i really wasn't too clear on what u were sayin lol. i didn't know that permanent insanity was the equiv. of death in d&d, tho, and also, my phb v. 3.5 says that greater restoration, heal, limited wish, miracle, or wish can all restore sanity to a creature... sooo...i'll stay tuned to see if i can understand u better.
