This idea is kindof abstract, isn't really good as a game mechanic more as a choose at your own disgression kind of rule. Why not more ill-effects when someone is attacked? Here's some examples.. Someone is struck with a sickle, now obviously this is a major wound, I don't know, there's a 5% chance it struck a vital area and now the PC is bleeding temendously. Taking 1d4hp loss every round untill the HP loss exceeds his constitution score, then if he didn't die, just falls unconciouse. A hit from a weapon can cause daze or even prone sometimes, might want to make will/reflex saves to make it somewhat balanced. Weaker spells like burning hands, because of the way they launch fire have more chance of causing burning. It burns the flesh and deforms the face, leaving scarred tissue and taking a -1d4 on charisma skillbased checks untill their wounds are cured. Perhaps even if they fail a reflex save they catch on fire, thus taking 1d4 fire damage untill they make a succesful reflex save to put themself out. (Or alternatively find some water) This is less damage then a normal encounter with basic-fire which is 2d6 damage. How do these sound? Note: I used d4s cause they seemed appropiate without being overly mean to the players. |

The PCs are stronger because TPKs suck alot. And, lastly, there are variants a plenty out there, both D&D brand and non, that give you ways to apply damage to specific areas of the body. |
