Two words: I'm bored.

I'd love to post my own but as I've not got any of the D&D stuff I can't play it. :(

Then again, I could make my own, so:

1. Being stuck down a deep anti-magic well with no magic items.
2. Having that Drow priestess come on to your character.
3. Forgetting to give your character clothes and walking into a town, completely naked (thanks, Real Life!).

6 - You journey for weeks into a vast underground complex to complete a ritual that will keep a portal to the demon wastes shut for another 1000 years and as you reach the alter you realize you left the artifact of pure goodness needed to complete the ritual at home in your sock drawer.

Destroy-the-world blunders are cool, but save-the-world blunders can be even better.

A friend of mine was DMing a Ravenloft campaign once, and over the course of the adventure the PCs wound up getting turned into vampires. Because of the thick clouds covering Ravenloft this wasn't as much of a problem as it might otherwise be, so the PCs didn't bother to get changed back.

The only problem was, their goal was to break the curse on Ravenloft. Eventually they succeed at this, and so everyone's happy, and evil is vanquished, and the sun comes out from behind the clouds...

...and the party gets TPKed by the happy ending.
