My game group has discovered:

Spiked Chain
Dex of 16
Combat Reflexes
Enlarge spell

Does this combination equal 2 AOO as the enemy closes to 5'? one AOO for each 10' passed?

No, unfortunately for them, it does not.

If a character is wielding a weapon with reach, that character may make ONE AoO on an enemy trying to move inside that reach. Once they're inside the reach, thats it, no more AoO from reach.

Also, if your weapon has a 20' reach, i do not believe that you can attack anything INSIDE that reach. You must move out to your reach. Of course, correct me if i'm wrong on this point.

I think the rule is stated in a way that you only get one AoO on any SINGLE individual on a given turn; however, with combat reflexes, you get more than one AoO, each one has to be on a different enemy. I don't believe there is a feat or situation that allows you more than one AoO on a single individual.

I belive that you are incorrect, The_Cheat137. Players Handbook 3.5, page number 137 states that you provoke an attack by leaving a threathned square. Thus, someone with a 10 foot reach, extended to 15 by using a spiked chain and thus able to attack and threathen at the 15, 10 and the 5 foot line (though only the 15 and 10 line is important here) that means that an opponent that charges said char will provoke 1 attack of opportunity when entering the 10 foot line (because he left the square at the 15 foot line) and then another one when he gets next to the char.

yes, it is a very, very bad idea to charge a foe who is set up like this, try to find a way around it by such feats as mobility or even spring attack, or monsters with similair abilities or reach.

If you move out of a threatened square and into another threatened square, you provoke an AoO. Moving from 20' to 15' will provoke an AoO so long as someone threatens (can attack) the 20' and the 15' squares. And since a spiked chain can attack adjacent and reach squares, such a character could do that.

In 3.5, you can, in fact, take more than one attack of opportunity on a character. However, I believe that these attacks have to be due to different provocations (ie. only once from movement, but another if they try to cast a spell or grapple you, etc).

Oh, wait, you don't mean the Metamagic Feat... You mean the spell Enlarge.

Now, everybody check their PHBs again.

Moving out of more than one square threatened by the same opponent in the same round doesn't count as more than one opportunity for attacks of opponent.

PHB pg. 138, the penultimate sentence in the section "Combat Reflexes and Additional Attacks of Opportunity."

Not quite, but close. You get one AoO for each opportunity. When an enemy character moves, it's one opportunity. No matter how many of your threatened squares he passes, it's still just one opportunity. However, if he provoked an AoO by moving, and then by casting a spell, then you'd get two attacks of opportunity.

Regardless of circumstance, you may only make 1 attack of opportunity against a foe in a round. In addition to this, if someone enters your threat range with the intent of attacking you, they do not draw attacks of opportunity.

yes just one AoO per move and the spiked chain is a special expception to not attacking within your threat range. a long spear couldnt but a Spike Chain can. :)

Anyone leaving a square that you threaten provokes an attack of opportunity. That means that, if he wants to hit me in the face or give me a kiss, he still provokes an attack of opportunity from me if I have greater than 5 feet of reach. With only 5 feet you only enter a threatened square and don't leave it.

You only get one AoO per character per "opportunity" not per round. Movement is one kind of opportunity. You only get one AOO for movement, no matter how many squares that you threaten that your opponent leaves. If you're opponent moves and provokes an AoO and then starts casting a spell and also provokes an AoO(You can't cast while threatened) then I get TWO AOOs on him if I can. The movement was one opportunity and the casting was a second.

everybody pro-multiple AoO need to cite some pages because the limited-to-1 AoO people are supporting their positions with concrete references. And no one can win an argument solely based upon 'because I said so'. :teach:

Combat Reflexes and Additional Attacks of Opportunity: If you have the Combat Reflexes feat you can add your Dexterity modifier to the number of attacks of opportunity you can make in a round. This feat does not let you make more than one attack for a given opportunity, but if the same opponent provokes two attacks of opportunity from you, you could make two separate attacks of opportunity (since each one represents a different opportunity). Moving out of more than one square threatened by the same opponent in the same round doesn’t count as more than one opportunity for that opponent. All these attacks are at your full normal attack bonus.

THere I put a quote that proves BOTH points!

after additional reading, i agree with some points brought up here.

You MAY make more than one AoO against an opponent in a round, just only one per opportunity.

If an opponent takes a move action, through any square you threaten, you get an AoO. However, the five-foot step included in a full-round action does not provoke an AoO

And attacking within a threat range seems to be possible only in special cirucumstances; on a weapon-by-weapon basis

This is the 3.0 method of taking an AoO. In v3.5, you may take one AoO per opportunity. Movement is considered a single opportunity, so only provoke once no matter how many squares an individual threatens.

If you want quotes, read the others in this thread or check the SRD. Right now I'm too lazy to find the exact reference and copy & paste.

If you have this feat you have your 1 + dex modifier AOO's per round, but you can only ever make ONE AOO PER given opportunity per creature you threaten. Moving is ONE opportunity, casting a spell is another, drinking a potion is another, getting up from prone is another, attacking with a ranged weapon is another, load a cross bow is another, pick up an item is another, read a scroll is another, etc (the list goes on and on).

There, clear as mud.

Because technically, tossing caltrops on the ground is not an attack, it's an action. You don't make an attack to use caltrops (though granted, they make an attack on their own if someone steps near them).

First of all, Spiked Chain, like many other reach weapons, does not donut, out. So it can attack people in the 5ft, or 10ft area.

Second of all, the rules changed from 3.0 to 3.5

In 3.0, even with combat reflexes you could only get one attack per person.

In 3.5, with combat reflexes, you can get multiple attacks per event, but all movement in the same round is considered one event.

But with 20ft reach, you probably are going to be using up your dex's worth of AoO's anyway, so no problem.

I always bring this up when people talk about AoOs ...

Drinking a potion, casting a spell or doing a variety of other tasks provokes an AoO.

Being helpless does not.

For my own clarification, is it all movement in a turn or is it each instance of movement? For instance, if a PC is under the effect of a haste spell and he moves through your threat range, attacks and leaves your threat range, is that just one AoO or is it two?

Your example would only allow one AoO from the movement. Moving out of more than one square threatened by an opponent in a round = one attack of opportunity. It doesn't differentiate between how or when the movement occurs in that round, or if you do things in between movement.

I don't know what the answer is if the person actually leaves all threatened squares, then returns. Literally, there isn't any additonal AoO.

BTW, in 3.5, haste doesn't give an extra action, it just bumps up your move speed. So, you can't move, attack, then move w/o spring attack or an equivalent.

This isn't quite right either. Haste gives one attack if and only if you made a full attack action (and the extra attack is at your full attack bonus). If you only made a standard attack, then you don't get the extra attack.

That's right. For whatever reason, I thought that it granted you an extra attack or an extra MEA. I musta been tired last night...

Umm, I thought that was a give-in since the only time you can make multiple attacks is with a full attack action...

but I can see where people could make that mistake.
