My DM is busy and i would like to help out by making a Questionnaire (or questions) for me to use. It would be very helpful...

A little more information would be helpful. What kind of questionnaire? What are you trying to find out?

This is a copy of the basic question sheet I gave my current group when we started a new game together. Note that this sheet is meant to be used as a guideline when writing a PC history. As such, these questons are not meant to be answered like a test so much as checked against the history to ensure that the history answers them all somehow.

These questions do not need to be answered as a list. I would much prefer if the answers to these questions could be found within your character’s history. Please see the attached character history example.

1: Where does your PC come from (this can be as specific as “From the Du’Caldia family in the village of Shirewood on the outskirts of the Forest of Randa along the King’s Road” or as non-specific as “From a tribe of nomadic sheep-herders in the hills to the North.”)? What made you leave this place?

2: Does your PC have any family or close friends? If not, what happened to them? If so, how do they get along with the PC and how often does the PC see them?

3: Is your PC related to or otherwise on a first-name basis with anyone of import (this would include the duke of a realm, the captain of the guard in a large city or a national hero, for example)?

4: Does your PC have a mentor or other close teacher? Is this mentor still alive? If so, where does this mentor generally reside?

5: How did your PC develop into the character class he/she is today, meaning what events led to his choice of profession (example: Your uncle, the black sheep outcast of the family, secretly taught you wizardry during your teenage years. When you were 17, your father found out and banished you from his house. You took in with a group of caravan guards, but longed for a chance to gain more magical power…a chance which adventuring can give you.)?

6: Does your PC have any notable rivals or enemies? If so, who are they and what is the problem between them and the PC?

7: Does your PC have any special quirks or strange/interesting habits that are worth noting (examples: You’ll go far out of your way to track down and kill aberrations because you consider them to be anti-natural or you collect gruesome trophies from worthy opponents you kill and taxidermy them as a decoration for your home or you can never sleep in a place unless you walk around it in a circle four times first or you have an active and possibly racist hatred for dwarves.)? Is there any reason behind your quirks/habits?
