A few weeks back our human swashbuckler & dwarven fighter got bored waiting for the rest of the party to winch themselves down to the cavern floor of a dungeon. (They're on the cavern floor, the druid & his wolf are being lowered down, & because they're the strongest, the human fighter & lizardman barbarian are up top working the winch)
So, instead of just sitting tight & controlling the landing zone, they decide to venture off down a side passage & "explore".
They went down the passage and discovered a closed set of double doors.
Wich they proceeded to open.
Inside the chamber they found an otyugh. Niether the players nor their characters had any idea of what the thing was....

So of course they charged it.

A few rounds in the druid/wolf join the fray, helping to dispatch the monster.

Unfortunately the fights been loud & attracted all kinds of attention....

So by the time the lizardman arrives he's dropping into the middle of a giant FUBAR melee & the entire cavern is on high alert. So much for the stealthy aproach....
Worse, they party is routed & forced away from the lower winch mechanism. Thus leaving the human fighter stranded above & in search of an alternate means down....

Ultimately, because the element of surprise was gone, this led to the lizardman & the swashbuckler meeting their deaths.

We had just started the WLD, so we check one of the doors in the first safe room for traps, find it untrapped and unlocked. We open it and see utter blackness. After much discussion we decide to tie a rope around the paladin and let the barbarian half-orc hold the other end. The Paladin walks through and vanishes, 10 seconds pass and the dwarf who is an impatient fellow barges through all of us and enters the doorway. Seeing him disappear we follow suit leaving the barbarian behind. He drops the rope and enters, at this point we have no light source and the paladin is a human so he has no idea we've entered
and we happen to go the complete opposite way he did. We had down and encounter some orcs and the Pally, playing true to his character, because obviously the guy sitting next to us knows we entered, runs blindly down the room screaming "WHO's THERE? STAY BACK OR YOU"LL TASTE MY SWORD" and runs smack into the back of an orc, knocking it and himself unconscious. The orc is the coup de graced by the fighter and once we woke the pally up he was quite ****** off.

We have a player who plays Half-Orc Barbs or Dwarf Fighters and hardly ever lets the Rogue check a door for traps. They want to plow threw every door like a Rhino even when they get hit with arrows, poison darts, etc. The cleric always bails them out.
