Quick question - if a character were making a full attack using two weapons, would multiple attacks due to a high Base Attack Bonus also apply to the offhand attack? Or are those purely for a primary attack? This problem has been nagging me for ages, and I can't seem to find an answer in the books.

You would still only have 1 extra attack when weilding 2 weapons, no matter your base attack bonus. However, you can increase the number of secondary attacks you get with Improved TWF and Greater TWF (TWF = two-weapon fighting).

You don't get the extra attacks with your off hand from having a high BAB. The only way to get the extra attacks is with feats (ie. Improved Twp-Weapon Fighting and Greater Two-Weapon Fighting).

Right on the button. But if you take imp. TWF, you get 2 off hand attacks. Greater TWF grants 3, provided you have the BaB for 2 and 3 attacks, respectively.

At the indicated penality, until you get Perfect Two Weapon Fighting which gives your off-hand the same attack sequence as your primary (both being subject to the normal TWF penalties).

Does anybody know that if you are using your shield as an off-hand weapon (shield bash), do you get the additional attacks from improved two-weapon and greater two-weapon fighting?

An off-hand weapon is an off-hand weapon, whether it be shortsword, longsword, or shield bash.
