So I suck at making campaign settings. Thus I've chosen to convert the region of Kanto to my will. This is what I have so far:

The adventurers start off in a small town by the sea (Pallet Town), where they are sent on a fetch quest to a nearby city (Viridian City). The path between the two is hilly and occassionally known to goblin/kobold/whatever ambushes (Route 1).

Having retrieved the quest item and returned it to the quest giver (Professor Oak), the quest giver makes a big revelation and proceeds to send them on the main quest arc.

Returning to the nearby city, the adventurers have their first encounter with our main reoccuring bad guy (Rival).

Having defeated the MRBG, the adventurers proceed through the nearby forest (Viridian Forest) to the next city (Pewter City). Here they learn vaguely of some ancient events and dormat creatures (Fossil Pokemon), as well as being issued an optional side quest (Pokemon Gym).

To get to the next area (Cerulean City), the adventurers have to pass through a cave system (Mt. Moon). However, a rock fall causes the adventurers to have to detour through one of the villian organisation's (Team Rocket) base.

Having reached the city, the adventurers are sent to retrieve an item from a nearby hamlet (Bill's House). Along the way they have our second encounter with the MRBG. Once they've successfully retrieved the package (S.S. Ticket), they head back to the city only to find an attack going on by the villians.

...

Aaaand that's as far as I've gotten. Obviously it needs a lot more fleshing out (ideas welcome), and it isn't finished, but from this point forward I'd like the story to naturally evolve rather than follow the route of the game.

Yeah, I was about to say, don't expect this to flow like a pokemon game. Kind of at all. Anybody with common sense will start deviating from the game path.

If it isn't the party's quest to go from town to town beating gyms or purifying the large shrines overrun by monsters, you'll have to devise different goals for the main quest or reason to seek out the towns. Item delivery, fortunately, is just item delivery.

I assume you're replacing pokemon with not-necessarily catchable D&D monsters, but I'm not sure to what degree you're planning to use the game maps. You could find blank dungeon maps online and DMG-randomly fill them, if you like.

Pokemon runs differently from D&D, but what I've found useful for applying to D&D is:

  • consideration of terrain/weather threats to PCs: falling or fallen boulders, snow piling up and hiding entrances or slowly freezing the party, mud pits swallowing up parties, mounts, and wagon wheels, and floors that collapse and deposit you on another level of a dungeon.
  • monster patrols you can hear or see on their route before you come face-to-face: you can then decide how to avoid, hail, or attack them.
  • finding/procuring new stuff by running into normal monsters or other NPCs, who might trade you for something you might already have in your pack, or demonstrate a new weapon or potion in battle against you and drop some extras later. Perhaps the world has many adventuring parties roaming about, but some prey on others, and so developing a party strategy to counter other parties' strategies is part of the game.
  • Summoning monsters via talismans can be fun, whether you find one-use talismans lying around or buy blank talismans to capture new monsters. I'd model it on the sleep spell if you want that "weaken the monster first" feel.

I can't help you much plot-wise, but in case you want to really make it Pokemon, I made a class for that.

No no no. As I said, I'm converting the region of Kanto to 5e. There will be no trainers, no Pokemon. The coties, villians, and people of power will remain the same though.
