Lair Action on Initiative Count 20

In the DM Basic Rules it says a Red Dragon can take a Lair action:

 

On initiative count 20 (losing initiative ties), the dragon
takes a lair action to cause one of the following effects;

 

 

My question is simple is initiative count 20 the first turn in the round or the last turn? And what does losing initiative ties mean in this context?

It's an action that happens on Initiative Count 20. If a PC or the dragon has Initiative 21 or higher, they go before the lair action. If a PC or the dragon has Initiative 20 exactly, they go first, then the lair action.

Creatures with initiative results 21+ act, in order.

 

Creatures with initaitve result 20 act.

 

Lair Action happens on 20 (having lost the tie-breaker to other 20s).

 

Creatures with initative results less than 20 act, in order.
