I've made a sheet to track a lot of the vital stats for parties. You can find it here:

Feedback welcomed!

Not bad, but at least for my own personal tastes it has a lot of stuff on there that I don't need.  Really the only stat I find I ever need as a DM is passive perception, not really anything else that I need to use secretly so I just get the info I need from the player as it's needed.  I also find it useful to have the players background personality traits, ideals, bonds and flaws handy to help with passing out inspiration.  And a small notes section so I can track long term things like curses, diseases etc that the player may not know about.

I try to keep the amount of stuff I need behind the screen to a minimum as table space can be an issue.  So I just use my laptop with adventure notes, and a table of the above mentioned character info in OneNote, a print out of the adventure, my dice, tokens/minis, and a small white board for tracking monster/npc hps and conditions.  I track initiative with colored clothes pins across the top of a screen.

That's a great idea! Thanks for the feedback Lawngnome4hire! I think tonight I will change it up so headers run along the side and add the background traits. I'll probably also strip some things out that already exist on the combat card I made, really no need for the overlap.

I usually keep track of Perception, Insight, Investigation, and saving throws, since those are the most likely things for me to roll in secret (secret mind control, searching for traps, telling if an NPC is lying, etc.).
