I had the impression that the last name of most house members should be "d'" plus its house's name, at least officially. However, when I was reading "The Lost Forge", I found the House Cannith agent had the name of Elaydren d’Vown, which made me confused.
Could someone give me a clearer summary of the naming conventions please?

well even thought they use the house name as a lastname but the smaller family have there own lastnames and these are become the middle name. so the person could be using there middle name as the last name.

Something of a misnomer is the idea that if you're a "marked" member of a draconic house your name ends in d'whatever. This actually isn't true, the names of the dragonmark houses simply reflect what was the then (during the War of the Last Mark) powerful dragonmarked families before they shacked up together and started running the economy of the country. The important thing to remember is the a d' prefix indicates whomever you're rubbing shoulders with is a dragonmarked scion, not just jo-schmo would-be-important. For example House Sivis has the Sivis family and 11 other families (the Torralyns, Severins, Lyrrimans, Syrralans etc). So a gnome with a name like Gedric d'Syrralans is gonna be a dragonmarked scion of House Sivis (makes guessing between more populace races like Humans and halflings more interesting...).

Unmarked scions of the House (either they don't have a mark or it just hasn't appeared yet) use the dragonmarked house as their last name (or obviously one of the families beneath it), but without the d' prefix. Again our example is Gedric Syrralans, of House Sivis (he would be quick to point out, or he's wearing a symbol of the House).

Hope that helps you out (gotta admit that one stumped me too when I first came across it)

Thanks for the replies. They're absolutely helpful.
Has the naming rules been specified in some book? Dragonmarked maybe?

Page 48 of Player's Guide to Eberron has a sidebar on "To d' or not to d'" which largely suggests that they use the family name with the d' prefix when dealing with those outside the family.

The sidebar CraggleRock mentions is the key. She's Elaydren Vown of House Cannith. As a member, of a dragonmarked bloodline, she's entitled to use the d' prefix. Among friends, she's likely to use d'Vown, because it's her personal family. Among strangers, she'll use d'Cannith, because it's a name people recognize - most strangers don't know what house Vown belongs to.

For example House Sivis has the Sivis family and 11 other families (the Torralyns, Severins, Lyrrimans, Syrralans etc).

It's worth noting that the Zil gnomes actually have a tradition of using all three names - personal, family, house - hence the matriarch is usually refered to as Lysse Lyrriman d'Sivis.
