I would make the Pyramid of Shadows a prison built by Aernal elves, used for their most dangerous criminals, especially those who became Ascended Councilors, and therefore are extremely powerful. The Pyramid is bound to Mabar like a man-made manifest zone, even though it's physical location is on Aernal. Although, and especially with Karavakos using a ritual to make it happen more often, the Pyramid will occasionally find itself in Mabar manifest zones. Most of the encounters remain the same (creatures bound either by the elves like the dragon, or those who wandered into the Pyramid and were unable to escape like the Ettin), although instead of a Tiefling and Eladrin, Karavakos and Vyrellis are instead both Deathless reduced in power from the loss of Irnia's manifest zone.
