If you want to see what is officially known about the early days of the Five Nations, check out the Grand History of Eberron. That should give you at least a starting point in the official lore. I would argue that the early nations were actually city-states that became the capitals of proto-nations, but still possessed central control over the other towns and city-states in their area of influence. Think of theearly Five Nations as similar to the American colonies in the 17th century, you get one major settlement, say Boston, from which other setttlers slowly branch out and found other settlements and towns, but they are all still loyal to the entire Massachusetts Bay Colony. Likewise, after founding say Korth, the human colonists of Khorvaire moved on to found Atur and Karlakkton, but still retained overall loyalty to the Karrnath nation.
