I've been thinking about the Thousand Yard Stare for quite some time.  Honestly, I'd never heard of them until I saw the background on the Character Creator, and my friend (that's as more well-versed in Eberron lore than I am) hadn't heard of them either.  Has the group ever appeared elsewhere or is it sort of open?

It's something we added in in 4E, along with organizations like the Fading Dream. In theory it's always been around, but in practice I don't believe you'll find it mentioned anywhere outside of the 4E books.

I only have 3.5 books, so I must ask; who and what is the Thousand Yard Stare?

They're a 4th edition group as far as I can tell.  They're House Medani operatives patrolling the Mournland's border with Thrane and Breland.  They see some messed up stuff, hence the name.

Ah, thanks HC.  That's sort of what I thought, but I was unsure.

I really like the idea of the Thousand Yard Stare... maybe made up largely of Balinor or Dol Arrah paladins specially trained to fight aberrations and other Mourny things.

I love it because of the name; it's evocative of those who have Seen Too Much.

Look at these two definitive pictures of the Thousand Yard Stare, the first a painting of a Marine at Peleliu, and the second, a photo of a Marine at Eniwetok:

Members of the Thousand Yard Stare could also make decent guides into the Mournland, since they probably have to go chasing after something through the mist every now and then.  Even if it's rarely, members could still point you to that one sergeant who's never quite been the same after hearing his buddy disappear when they chased a group of warforged terrorists into "the grey".

And as the OP's Eberron-loving DM, I'm interested to see why his character handle functioning in civilization while away from the mists.
