In the back of the ECG, there is an NPC, Lord Major Bren ir'Gadden.

What is a Lord Major? Is it a civlian title, google insists on Lord Mayor as correction, or a military title?
If military, is it a Major, or something higher such as a General?

It is whatever you want it to be.

That said, I think you are amalgamating two titles: "Lord" and "Major".  He holds the rank of Major in the Brelish armed forces. However, he is of noble blood, so he also holds the title of Lord. If he got promoted, he would be a "Lord Colonel." Edit: A commoner of equal military rank would simply be, "Major" or "Colonel."

I'm in agreement with Ogiwan. That's how I interpreted it.

The Lord Major is still playing a role in our campaign as a patron of sorts. We're level 3, and I suspect we may soon outgrow him, but who can say?

The Lord Mayor Bren ir'Gadden still has a place in my campaign, albeit not a large one at the moment.

His help in the early combats and my outrageous die rolls in his behalf have even earned him a Sharn wide nickname.

Lord Mayhem!

He hates it when people (especially his subordinates) call him that Innocent But that's what you get for slicing through enemies like a Flaming Fullblade through paper.

I posit the existence of Lords Major and Minor. Breland is a proto-democracy, perhaps both upper and lower houses of parliament consist of nobles.
Some, the Lords Major, are the powerhouse political families of the nation. The Dukes and Earls and Barons.
The others, the Lords Minor, are Counts, Baronets, Knights, and Squires.

That's my take on it at least.
