I need any info on any established portals in Faerun for my campaign. I need info on where a portal is located, where it leads to, and if there are any tolls, certain circumstances, or expenses that would prevent my PC's from using it. Any info would be greatly appreciated

This sounds like something that will be campaign specific.  So there is a portal as close/far away as is convenient to your story, and the problems in using it will be just enough to test your party.

Yeah, compilling a list of all known Portals in the Realms would be quite an undertaking, theres alot! Read some of the Elminster novels, hes all the time using portals. There was also quite a few of them in Myth Drannor, and... well theyre everywhere!!

David has it right, feel free to put a portal anywhere and everywhere you want, most portals in the Realms are made so that no one would even know that its there, unless its activated, by hopping on one foot during a full moon or something.

There is an archive of some portals that they did awhile back, back when they was doing more Realms web articles, sigh, I really miss those : www.wizards.com/default.asp?x=dnd/archfr...

Whether or not any of these have survived the Spellplague is up to you.

If you have access to it, there was a list of some of the prominent portals in the Realms in 3e's Magic of Faerun, though the list is far, far, far from exhaustive.  As others have said, you're probably better off coming up with your own.
