When I first read 4e I enjoyed the idea of being able to allow the players to ask if the monster or NPC is bloodied, or for the monsters to know who is bloodied among the PCs. However I wanted a way to tell the players that the monster is not bloodied yet but was still hurt, or was beyond bloodied and almost dead. So I came up with this:

Once the monster reaches 75% of its health it is "Bruised".

at 50% it is "Bloodied".

at 25% it is '"Severely Bloodied".

So far it has worked great and we have had no problems adding it to the game. It has expanded the tactics for both the players and the monster/NPC groups.

It's a great idea, the only problem is the additional bookkeeping. However if it works for you, that's awesome.

I do something similar, but I use descriptions based on the amount of hp, and just sort of eyeball it.

Hmm... That is another idea that I may have to try. What kind of descriptions?

So long as there's no mechanical change, go nuts. :P

We use the video game Neverwinter Nights I conditions--barely wounded, wounded, barely injured, injured, seriously injured (i.e. bloodied),dying, dead. If their powers require them to know if the opponent is bloodied, or if they just really want to know, I let them roll a Heal check DC 15 as a free action. Otherwise I keep the conditions from them but still apply their bonuses. I make it a point to add that their attack "seemed to do some serious damage" or make up some other greivous wound they've applyed to their foe.
Hope this helps.
