Hand Item Action?

I was wondering about how this is already handled and how I might handle it.  As it stands, there exists no action for "Give ally [blank]", either throwing whatever you're going to give them and handing it to them (and yes, I mean in the middle of combat).

My idea was this:

Hand Item

Action: Swift Action                   Range: Melee touch or Ranged (whatever the maximum range for a thrown improvised weapon is)

Requirement: You must have the item you want to give in your hand.

Effect: The item is now in the available hand of the person you gave it to (i.e., they catch it as a free action), or if they had no hand available or were unwilling (for "Here, monster, catch this" and it's a bomb triggered by touch or somesuch), the item is on the ground in their space.

...

However, it may be that this new rule is just too clunky and tries to mechanically define what is often handled by hand-waiving.  I don't know.

Yeah, I generally handle this by handwaiving. Also, I don't think Swift actions exist in 4e, so I'm guessing it'd be a minor there?

I think from the dozen or so times this has come up before that the general consensus is a character can take something from another character's hand or put something in another character's hand if they are adjacent and it's a minor action. If you need to toss something to someone, then one player has to ready an action to either catch or toss and the one with the higher dex can make a DC10 dex ability check to avoid missing. If the toss is say more than 3 squares then make it a DC15, and between 10 and 20 squares make it DC20.
