Seeking a planes-hopping LFR mod

I'm building to a homebrew 4e Planescape campaign, and would like a good jumping off point.

 

Are there any good LFR mods that end, or feature, popping in and out of portals or unique dimensional locations? A good mod helps me handwave reasons why a diverse party would assemble, so I was hoping to save a little work by getting some recommendations.

 

Could anyone offer up their favorite experiences?
