Isn't Nentir Vale a part / spinoff of the old Greyhawk campaign worl setting?

I really liked the Nentir Vale. Kept me buying 4e long after I lost interest in the rules.

Would probably take fourth place for settings I'd like to see printed for 5e

The DMG has the Points of Light Gods and their domains, but I like yours better. The Evil Gods are tricky because the domains don't fit them as well. But then I would say the Cleric class as a whole is more centered around working Good than Evil.

Fortunately with the Acolyte background on other classes you get a good "Priest" of an Evil God. For instance the Acolyte background, with Wild Magic Sorcerer, Great Old One Warlock, or Way of the Four Elements Monks all would make a better priest of Tharizdun than a Tempest Cleric. A way of Shadows Monk or a Moon Circle Druid who choosese only scaly shapes makes a really great devotee of Zehir. Making a Cultist background as a Charlatan variant would be a good way of accomplishing this as well.

Of course all of this is only necessary if your Players actually want to play one. As for the NPC's in the back of the book, the Acolyte and Priest swapping out spells for ones that feel more like the evil God's  domain you don't need to use the domain rules to accomplish.

I really don't think much is needs to be converted to play a campaign in this world.
