In reading d20 Modern, I have not seen a mechanic like this before. Once a person rolls for a successful hit and damage, the other person can just shrug it off? But I can see how this could realistically play out. There are people who are run over by cars who walk away without a scratch. So, a hard hit doesn’t necessarily mean damage. On the other hand, I can’t imagine taking a solid hit from a gun (especially a shotgun), and being able to shrug it off.

Anyway, I’d like to hear from people who have played with this mechanic. How does it compare (in gameplay and feel) to other damage mechanics with which you’ve played?

There is not a Toughness Save and there is not a mechanic in d20 modern that works in the way you suggest. You get hit = you get damaged.

There is a Fortitude Save and it works just like well the Fortitude Save in D&D, it is the exact same mechanic.

There is a massive damage rule that forces you to make a Fortitude Save if you take damage greater than this amount, fail and you are dropped immediately to -1 hit points, succeed and you TAKE damage normally. Your massive damage threshold (MDT) is equal to your Constitution score + any feat/class modifiers.

The Tough Hero can take damage reduction and energy resistance, this is similar to the Barbarian (D&D).

Toughness is a Feat that gives a one time +3 hit point bonus.

Evasion feats/abilities can be used to make Reflex Saves for no damage instead of half damage (you get out of the way), but this isn't toughness or fortitude.
