Need help making a plague doctor!

As i've already said in the title i've recently come up with the idea of playing a plague doctor character. The thing is i want to be evil so i'm thinking that the character will most likely pose as a plague doctor but in fact he will be something like a mad scientist trying to find new victims for his experiments. So instead of trying to cure the plague or another disease he will murder people and do other heinous acts. The problem is i don't know which class to choose for this kind of concept. I know i'll have to have a kind of high charisma mod in order to trick people using deception and such and i want to be a caster using spells that poison people or lacerate them or other doctorish stuff. I definitely don't wanna shoot fireballs and call thunder tho. Any helping in choosing a class is appreciated, i also don't mind multiclassing if anyone comes up with an interesting proposition.

Charlatan background, with Scheme/Scam #3, then Cleric/Wizard specializing in Necromancy is the obvious one.

There's really only 1 disease spell which is contagion.  It's only avalible to clerics, druids, and bards (via magic secrets).

Bards have charisma, and can convince people easily.

 

Warlocks might work too.

 

Edit: Ray of sicknes, is avaiblie to warlocks and sorcerer's.  Both Cha based.

If you go lore bard, you can pick up ray of sickness at 6, and contagion at 10.

 

I could see reflavoring some other spells too.

 

Crown of Maddness -> Rabies

Phantasmal Force -> Feavered Dreams

Bestow Curse -> Bestow Sickness

ect...

Cloudkill would be a good spell to consider, too, available to sorcerers, wizards, some land druids, and bards.

Death Domain Cleric possibly with some GOO Warlock levels. Or maybe a Fiend pact with Orcus. Charlatain trait would be useful as well, possibly with the Actor feat when you're pretending to be your good doctor alter ego.

I would go with the lore bard. CHA base to start with and skills to pull off the ruse, plus spells to cover the concept and spells that can also be thematic like mellored pointed out. They can also heal when needed to help maintain the cover. The other option might be an assassin with a healer feat.

For IRL inspiration, you and your DM have:

- News reporter (on medical matters!) who leaves quarantine to go get a cup o' soup - from a trendy upscale bistro, no less

- The doctor in NYC who decided he could roam around town until he suddenly got deathly ill one afternoon

- The nurse in Maine who has been more concerned about Her Rights than about protecting everybody else from a deadly contagious disease

- The first guy in Dallas who lied to the hospital about his medical condition - and spread it to two of the nurses helping him

- The head of CDC who cannot figure out why the healthy people want to keep the sick people under quarantine until cured

In fact, the CDC guy would make a very good prototype for your character.  But make him forked-tongue and malicious instead of just incompetent and behind-the-curve.  After all, your character is NE.

 

All that said, I do not recommend Evil characters because it too often spoils the game for your fellow players.
