What would u say (as a gm )happens when a creature gets impaled whith a longsword and then revives?

Firstly, creatures don't get impaled at all unless that is the choice of how you describe something which seems reasonable, such as if you decide a final blow - the one that actually kills the creature, not just reduces their HP or leaves then unconscious or dying - is an impalement.

As for "revives", that depends on what you mean. If you are talking about being brought back to life by spells like revivify, or raise dead, then what happens is that the fatal wound closes and the creature is brought back to life.

If you were asking what happens if a creature is reduced to 0 hit points by a longsword attack and then regains hit points, the answer is reliant upon the fact that such a scenario is not supposed to be described as an impalement in the first place so there is no question as to how that works out - the regaining of HP restores the creature to consciousness, and proves that whatever attack previously hit them wasn't a lethal blow.

But the OP is asking about being impaled lethally: he wouldn't have mentioned revival otherwise.

If its magical healing, your wound starts closing

If you just roled stabilizaed yourself with death saving throws, your wound is clotting or your clothing/armor is doing enough to keep your guts from spilling out.

its all dependent on how the "revival" happens, magical and mudane

so this is what happened:

We were fighting a feary dragon and we knew it would revive after a few rounds. So a friend of mine decided to impale him after leaving it unconcsious. (impale as in leaving the sword in there). So when the dragon revives our DM didn´t know how to rule this.

All we know was that the dragon regained hit points after a few rounds of being uncouncsious. But we know it wasn´t regeneration, it was an encounter specific mechanic. (the encounter ended when all feary dragons were dead at the same time)

OHHHHH. Pin it, treat it as grappled and restrained, strength check to get the sword out of its body. Its already a magical creature, and sounds like there was some extra magic going on at the moment, so I dont think you need to worry about things like taking points of damage each round, though your dm might decide that struggling to get the sword out does cause some damage. We all thought this was about reviving a PC

There is no impale condition. If you as GM make up an impaled condition, then you'd need to make up what happens if they can revive.

I don't have much interest in focusing on the body horror of impalement. So it's really unlikely to happen to begin with, with me as GM.

I didnt really mean a new condition, more, the creature is under the effects of being grappled, though the DC to escape is set instead of two creatures rolling against each other every round.

Though I will say that seeing you say you have no intreset in horor with your particular avatar is a little interesting.

My mention of taking damage was actually more to a refrence of the challange naspe said they where trying to complete. Every fairie dragon had to have 0 hitpoints at the same time. Depending on how a GM wants to rule, if its possible for the dragon to regen while "impaled", if they take damage cause I would assume the dragon would be attempting to get free since it seems like to me they where probably summoned by or part of some archfeys trial of one kind or another.

What was i trying to imply? Wou have an avatar that I would say could very easly be called horor themed, thats the only reason why I brought it up

In my campaign, only the Sword in the Stone is magically enchanted to stay put no matter WHAT happens around it.

If somebody uses magic on a dead body, the weapons fall out of the wounds so healing can happen.  (Unless the monster is some sort of golem or warforged, in which case - abandoned weapon moves to the same place as Wolverine's claws, while body magically is restored.)

I don't understand why you would ever impale and enemy and lose your weapon when could just as easily remove the head.
