Do sleeping NPCs get perception checks vs stealth? Had a game the other week and wasn't sure. I gave them disadvantage on their rolls, but that even felt like to much for someone sleeping.

No one sleeping or awake gets a perception check vs stealth. When someone is stealthing up on someone they make a roll vs. PASSIVE PERCEPTION. The only time someone makes an active Perception roll vs stealth is if they are actively looking for them.

Keep in mind stealth is very different in 5e as soon as you leave concealment you are seen. Now sleeping with no watch I would rule stealth check vs Pastive Perception -5 since that's the rule for disadvantage if your assigning a penalty to a static number.

The only time you need to roll is if there is doubt in the outcome.  For  decently sneaky/careful person coming into camp... you could easly decide no check is necessary.

The belief that you are automatically seen once you leave concealment is not supported by the rules.

Someone standing guard duty could easily warrant an active perception check... since that is what they are doing.
