Haven't found much use for this spell, but it could be a good all around combat buff, if some of the forms have good spell likes.
I have checked out most of the possible forms, but I dont have MMV or FCII to check out these:
Demon, Carnage (CE, MMV, evil caster)
Demon, Gadacro (CE, MMV, evil caster)
Spined devil (LE, FCII, LE caster)
Abishai, White (LE, FCII, LE caster)

These wont do me much good, as I generally play True Neutral, but I am curious. For myself, the best use I can see is bootstrapping ones way into Outsider polymorphing, by Fiendforming into a Bloodbag Imp.Each Imp has one or two forms it can polymorph into, must be medium size, max.
This would might be my choice:
Kelvezu (Demon); Outsider(evil); medium; Fly 60; NA +15; Str 21, Dex 31, Con 16, Int 17; MM2 p.60
Benefits: Poison(Ex): Injury Fort DC 19, 1d6 CON / 1d6 Con, Sneak Attack +8d6;

White Abishai and Spined Devil are not really worth it - straight-up low level combatants with little in the way of utility. I don't know the others, though.

On the contrary, the Spined Devil is a pretty excellent form for casters. All those abilities, for one 5th level spell!

Fly speed of 120ft!(average manuv.)
Immunity to fire and poison, resistance 10 to acid and cold
25 Dex
Small size

I had a master spellthief that it was pretty effective for, at mid levels.

Edit: I forgot DR 5/magic and telepathy!

My favorite use for fiendform is to cast it then cast polymorph to become an outsider.

If you are willing to limit yourself to medium sized forms, you dont even need the
polymorph spell. Blood bag, Euphoric, and Filth Imps all get to Polymorph as a 12th level caster. They are limited to medium forms, but I think you get to keep their Spell likes etc, when you be come something else.

I was looking at this as a potential Greater Anyspell target, and I think this is a great Buff, something to keep locked and loaded for Oh **** moments.
One level higher than Polymorph, and I get Invisibility at the minimum.

Soneillon, thank you, this is just the utility I was looking for!One spell that does so much, very cool.

Any one know about the other two:
Demon, Carnage (CE, MMV, evil caster)
Demon, Gadacro (CE, MMV, evil caster)

?

Demon, Carnage (CE, MMV, evil caster)
medium
dr 5/silver
str 20, dex 10, con 14
must make will saves each round or attack nearest non-carnage demon

Doesn't seem like a good form

Demon, Gadacro (CE, MMV, evil caster):
small
dr 5/cold iron or good
Sudden escape (pretty cool ability)
fly speed: 40 ft (PERFECT!)
str 7, dex 18, con 15

Could be nice: the fly speed has perfect manouverability, and sudden escape can be worth it if you don't have that PH2 conjurer substitution.
