28 + (WBL/27,500) + ((HD-18)/3)+ (HD-20)/2 + HD/4. This is my initial guess without looking into something with a racial bonus from PAO.

EDIT: At lower levels, 20 + sqrt(WBL/1000) + ((HD-18)/3)+ (HD-20)/2 + HD/4

EDIT 2: Revised base to include dragonborn mongrelfolk. Was lazy before and grabbed dwarf, it was handy. All credit for the base score (28) goes to Xoraxx, down there. Using his Dragonborn Mongrelfolk Barbarian 2/ Stoneblessed (dwarf) 3/ Dwarven Paragon 3/Something Else XXX.

EDIT 3: Probably something awesome using Primeval, PAO and the below build. I was feeling snarky earlier.

Puts you at +10 con (+14 while raging but thats a temporary effect)
Amulet of Health +6
Manuel of Bodily Health +5
level up bonus +5
