Am I crazy/blind, or is the PHB missing both Destructive Smite (5th Paladin) and Trap the Soul (8th Wizard)?

That's a pretty big omission, if so.

You aren't crazy and you are not blind. Those spells ARE missing.

I see "Destructive Wave" as a 5th level spell on page 231. Possible new name? "Trap the soul" is not in the spell descriptions that I can see. It is (according to page 211) a spell though. heh

Yeah, Destructive Wave might be a rename of Destructive Smite, since they are both 5th Level Paladin spells, but Trap the Soul... there's nothing else for 8th Level Wizard that fits.  Unfortunate to have such a big omission from the flagship book.

Well there are pro's and con's to having a first printing of a book. Perhaps Wotc will address the question and post the correct spells for us though.

I had a lot of troube with the spells in the PHB, a lot of them blend together leading to some major confusion on my part. I'm also seeing absolutely nothing that could be trap the soul.

Trap the Soul was folded into the 9th-level Imprisonment, pretty much, as one of Imprisonment's forms is trapping someone in a tiny gem.

That's my take as well:  Trap the Soul has been subsumed by Imprisonment (specifically the 'minimus containment' option).  That's what trap the soul does, imprisons the soul in a gem (check 3e version for example).

Fine print for errata typically states something like: spell descriptions are "primary sources", while spell lists are "secondary sources"; so I can understand how the name slipped by on the lists.

Personally, I think they should call that option 'trap the soul' in the Imprisonment spell itself.

Destructive Smite/Wave was called Disintigrating Smite in the leaked Alpha Document.  They probably decided it needed a new name, and then there was a miscommunication about what to do with it.
