I posted my sheet on Enworld a few days ago, and while it's gotten a good number of downloads so far, I haven't gotten much feedback on it. So I figure I'll post it here in the hopes of getting some discussion going.

Here is the description I used on Enworld:

About:
This is a modified version of the official 5e character sheet (2 pages). I re-arranged some elements to improve the readability and consistency. Very high resolution.

Specific Changes:
• Added place to put your character level(s) next to your Character Name. This makes it easier to show multiclass levels
• Changed the fields in the box to the right of Character Name
• Added room for Inspiration, Proficiency Bonus, and Passive Perception to fit without looking terrible like on the official sheet
• Made the Ability Score information in the Skills box grey so it doesn't stand out as much
• Completely re-arranged / re-designed the AC & HP boxes for usability
• Moved the Personality Traits boxes to page 2 and replaced them with a section containing Hit Dice, Death Saves, Initiative, Speed, and Vision
• Widened and added lines to the Attacks & Spellcasting, Features & Traits, and Inventory & Equipment boxes
• Re-sized and moved some stuff on page 2 to better match the style of page 1 (particularly the box outlines)

A note on how to use the Hit Dice box:
• In the left blank spot, you write the number of hit dice of that type that you have i.e. _4_ d6 _____ to show that you have 4 Hit Dice available.
• In the left blank spot, you use tick marks to check off how many you've used i.e. _4_ d6 __l l l__ to show you've used up 3 of your available 4 Hit Dice.

I've shown this to my group (and we use it when we play) but we haven't been able to play much (and certainly not at high level), so I'm curious how it holds up for different classes / levels. Let me know what you think!

I like what you did for the Max HP and HD boxes, but I don't need that amount of room for attacks. Tho' I know why you put it on the 2nd page, I'd put personality traits, et al on the front because I tend to put treasure with equipment and draw a portrait on the back without actually printing out the 2nd page. Anyway, looks good!

This fails basic character sheet design 101. It's not form fillable.

Group response is unanimous (compared to the 3-sheet 'original' from Wizards) and enthusiastic.  I didn't see the 'original' sheet you made, but this one you've presented here is a very nice 2-sheet that moves things to a much better set of locations imho.  I felt like there was way too much extraneous junk on the front of the original sheet but this one swaps around almost everything I'd consider extraneous for things that belong better.   Hope that helps some!

Another character sheet I saw that I liked had the relavent skills under each stat.  Made it very intuitive for skills, bonuses, etc.

Also, it would be nice to have a spot for spell DC and spellcasting bonuses in the spell box.
