Do you already have or were you given a DCI number ?
Do you know if the GM has reported the event yet ?
There are several fan made sheets out there as well as the one provided by the RPGA. I would recommend that you download and read thru the Character Creation Guidelines v1.8.

If you download and read the CCG v1.8 I recommended I bet you will find it there
Also if you join the Yahoo Group for the region that you reside in I would bet that they would have one there as well.Since you have registered your DCI number go and log into your account. Go to the WPN Private and WPN Public links for Characters and enter in your character name.
I tend to list mine like Ishkur (Stormsoul Genasi Swordmage) etc. Once you have more than 1 character you will want to remember which one is which. Because when you fill out the tracking sheet you will be writing in which character you are using.
