Is there a new Herald GM test for 4E?

There is. It's in the DCI/RPGA Judge Center, located at http://judge.wizards.com/

No. If you pass a Herald exam, you're an RPGA Herald. You're not a v3.5 Herald or a v4 Herald. You will not be required to take a new Herald exam if you have already passed one.
