Some spell like Feeblemind set the target Intellect to 1 |

Shadow can Strength Drain and the Intellect Devourer has Devour Intellect (INT damage).
A few more monsters have Life Drain and similar that reduce Hit Point Maximum |

I applied disadvantage on all ability checks and saves associated with the ability that is reduced. This application works okay for the most part until you come to Con damage. At that point I decided to use a reduction in maximum HP and disadvantage on all Con saves.
As an alternate route you may use the Exhaustion table to represent ability damage. The game session I ran in which all characters started with 3 levels was a heavy role play game. |
